Two things worth reading off that table. Why Semaglutide Causes Fatigue: Six Mechanisms
Semaglutide does not have a known direct sedating action on the central nervous system. Almost all of the fatigue is a second-order effect of appetite suppression and delayed gastric emptying.

Dose Escalation and the Fatigue Spike
The standard Wegovy titration moves 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg at four-week intervals. That schedule is a default, not a requirement, and slowing it is the standard clinical response to tolerability problems including fatigue.
Two levers most patients do not realize they have. First, holding at the current dose for an extra four weeks before stepping up is usually acceptable and gives intake and hydration time to normalize. Second, some patients do well staying at a lower maintenance dose indefinitely if weight loss is adequate there; the highest tolerated dose is not automatically the right one. What Actually Helps
Ordered by expected yield rather than by how often they get recommended:
- Fluid and sodium. Target roughly 2.5 to 3 L of fluid daily and do not restrict salt unless a physician told you to. Broth, electrolyte packets, and salted food all count. This resolves a meaningful share of cases within days.
- Protein floor. Roughly 1.2 to 1.6 g per kg body weight daily. At low food volume, that usually requires deliberate protein-first meals or shakes.
- Calorie floor. Do not run below about 1,200 to 1,500 kcal. If appetite suppression makes that hard, use energy-dense foods rather than trying to eat more volume.
- Keep some carbohydrate. Combining a GLP-1 with a very low carbohydrate diet compounds fatigue for little added benefit.

- Hold or slow the titration. The most reliable fix when the fatigue is clearly dose-linked.

Fatigue is one of the least specific symptoms in medicine, and starting a GLP-1 does not make you immune to the ordinary causes. Attributing everything to the drug is how treatable conditions get missed. Worth ruling out:
- Iron deficiency anemia and low ferritin, more likely with reduced intake, and common in menstruating women
- B12 deficiency, which develops over months of low intake and also causes neuropathy
- Hypothyroidism, which shares fatigue, constipation, and cold intolerance with GLP-1 side effects
- Obstructive sleep apnea, which is common in this population and often improves with weight loss but does not disappear

The 11% figure comes from trials of FDA-approved Wegovy. Compounded semaglutide has no equivalent data, because compounded preparations are not required to undergo clinical trials and are not FDA-approved or reviewed for safety, effectiveness, or quality.
Two structural differences matter for anyone trying to interpret their own fatigue. Compounded products are typically dispensed as multi-dose vials with patient-drawn syringes, and the FDA has received hundreds of adverse event reports involving compounded semaglutide, a portion of them dosing errors from exactly this setup. Additives are the second issue. A July 2026 JAMA Health Forum secret-shopper study of 75 clinics and med spas in West Virginia and Oklahoma found 58.7% of compounded offerings contained additives, most commonly B vitamins at 56.0%, along with levocarnitine and glycine. Same-day contact if fatigue arrives with any of these:
- Severe or persistent abdominal pain, particularly radiating to the back, with or without vomiting
- Sweating, shakiness, confusion, or blurred vision, especially if you take insulin or a sulfonylurea
- Inability to keep fluids down for more than 24 hours
- Dark urine with markedly reduced output, or fainting on standing
- Resting heart rate that has climbed noticeably along with the fatigue

Both list fatigue as a reported adverse reaction, and the mechanisms are identical because the mechanisms are mostly about eating less. Comparing raw percentages across separate trial programs is not sound, since the populations, durations, and reporting conventions differ.
What can be said: tirzepatide produces larger average weight loss, which means a larger average energy deficit, which is the main driver of fatigue.
